Pediatric Physical Therapy, Breast and Bottle Feeding Therapy

In order to best provide specialized, efficient, and effective care, we have chosen not to engage with any insurance companies for physical therapy services. Therapy-only sessions will be considered out-of-network. The session fee will be automatically charged to the credit card or HSA/FSA card on file on the day of the appointment. Upon request, we will provide a Superbill that you can submit to your insurance company for potential reimbursement.

Comprehensive Lactation Consultation

Dr. Kops is in network with most Aetna and United Healthcare commercial plans and will bill them directly for lactation sessions when both the lactating parent and infant have in-network insurance plans. Note: We bill insurance for both the lactating parent and infant who are part of the appointment. Lactation sessions are considered preventative services under The Affordable Care Act. If your insurance carrier assigns any portion patient responsibility (deductible, copay, etc.), we will automatically charge the credit card on file for that portion. You may choose to file an appeal per ACA preventative services.

Other insurance carriers are out-of-network at this time, but we are always working towards joining other major carriers. Out-of-network fees will be automatically charged to the credit card or HSA/FSA card on file on the day of the appointment. Upon request, we will provide a Superbill that you can submit to your insurance company for potential reimbursement.

Included Services

Follow-up assistance via text, email, or patient portal, for any concerns addressed during your session is included in the cost of the service. Follow-up care that regards new or unaddressed concerns, or is extensive in nature, may require an additional session.

Out-of-Network Sessions

Comprehensive lactation & feeding consultation – $250 (initial and follow-up)
Infant therapy for breastfeeding, bottle feeding, and TOTs – initial $250, follow-up $200
Developmental Physical Therapy Session – $150 (initial and follow-up)

Home visits in Alexandria and Arlington, VA

All services are currently provided in office in Kingstowne, Alexandria. We are not routinely providing home visits at this time.

Insurance: Deductibles, Co-pays, Co-insurance

Although The Affordable Care Act states that lactation services are preventive and not subject to cost-sharing, sometimes insurance providers apply a portion of either the feeding parent, or infant’s claim to cost-sharing. When this happens, we are required by law to bill you for the portion that is assigned to “patient responsibility”. Upon receipt of the Estimation of Benefits (EOB), your credit card on file will be charged.

Cancellation Fees

Your appointment time is reserved just for you! A late cancellation or missed session leaves a hole in the provider’s day that could have been filled by another patient. As such, we request 48 hours notice for any cancellations or changes to your appointment time.

If you realize you need to change your appointment time with more than 48 hours notice, please go ahead and reschedule via the patient portal as soon as possible.

Rescheduling with less than 48 hours notice will not be able to be completed in the patient portal. Please contact Dr. Kops directly to reschedule. These changes may incur a $25 administrative fee if recurrent.

For cancellations made within 24 hrs of your scheduled appointment time, we charge 100% of the self-pay rate of your appointment type. If a last minute cancellation is unavoidable, you are encouraged to reach out to determine if converting to a virtual or phone appointment would be appropriate.
